Consent to Collect and Use Child Information

By using this app, I confirm the following:

1. I am the parent or legal guardian of the child whose data will be entered into the App Pro Simple Soccer platform.
   
   
2. I understand that this app is designed for parents and authorized team staff to record and monitor on-field performance data such as:
   
   
   • First name
     
     
   • Team position
     
     
   • Attendance
     
     
   • Goals and assists
     
     
   • Player stats (non-sensitive)
     
     
3. I consent to the collection and secure storage of this limited data for the purpose of:
   
   
   • Team management
     
     
   • Player development tracking
     
     
   • Personal use within the app only
     
     
4. I understand that:
   
   
   • No sensitive data (photos, addresses, birthdates, etc.) will be collected
     
     
   • All data is securely hosted on Amazon Web Services (AWS)
     
     
   • My child’s data will never be sold or used for advertising
     
     
5. I may at any time:
   
   
   • Request to review or delete the data associated with my child
